Any licensed physician can perform liposuction, but it’s usually performed by plastic surgeons and dermatologists in their offices with local anesthesia, though it may be performed in a hospital under general anesthesia. No special training is required, though some doctors’ professional associations recommend it. When choosing a doctor, you may want to consider whether they have had specific training for liposuction and how many they have performed.
Aging brings on a general redistribution of body fat, especially around the middle. For women, childbirth can leave behind a roll of stubborn and unsightly belly fat. And, of course, genetics count for a lot, too. But when it comes to liposuction, not all fat is created equal. Fat that’s resistant to diet and exercise is usually subcutaneous fat, which lies beneath the skin and on top of the abdominal muscle wall. Although liposuction is used to get rid of fat, it’s not a weight-loss solution. Liposuction works best on deposits of fat that are concentrated in particular areas and resistant to exercise, particularly around the stomach, thighs, hips and buttocks. You might lose a little weight, but it’s not likely to be significant. Liposuction also won’t fix a bulging stomach that’s caused by weakness in the abdominal wall, and it won't tighten loose skin. Breast lift, or mastopexy is a surgical procedure used to raise and reshape sagging breasts.  Factors such as pregnancy, nursing, weight change, aging and gravity produce changes in the appearance of a woman’s breasts.  As the skin loses its elasticity, the breasts often lose their shape and begin to sag. If your breasts are small or have lost volume after pregnancy, breast implants inserted in conjunction with mastopexy can increase both firmness and size. There are a variety of different surgical techniques used for the reshaping and lifting of the female breast. Some people see lumpy areas after the swelling starts to go down. This should improve after a few weeks, but wearing a compression garment can help prevent lumps. Less commonly, patients can have a hematoma (a temporary pooling of blood under the skin) or seroma (a pocket of fluid under the skin that requires draining by your doctor); changes in skin color or sensation; or scarring from thermal burning, either above or below the skin (usually an issue only with laser- and ultrasound-assisted lipo, if skin ports aren’t used). Some women seek out scarless breast lift because they wish to avoid scars which are the hallmark of a breast lift.  Unfortunately, a true breast lift cannot be done with scars.  However, in some instances, a woman looking for a breast lift only needs more volume in the upper part of her breast and in such situation a breast augmentation helps to reshape the breast and creates the illusion of a lift.  This is a procedure that is appropriate only for a very limited number of patients as this is not a true lift.
However, minimizing the appearance of scars on the breast will be of vital importance to your surgeon. The whole point of a breast lift procedure is to make the breast more attractive, and conspicuous or heavy scarring defeats that point. Most scars will typically heal and fade within two years, but will always be present. Your surgeon will consider it a part of their job to keep these scars discrete, not a bonus. When skin is damaged, the body repairs it by creating new collagen fibers, which gives the healing area a different look and texture: a scar. The “mended” skin will be less flexible than the skin around it, and it may have no hair follicles. The scarred skin might be a different color than surrounding skin and the color will change as healing progresses. This is completely natural.

The general anesthesia (intravenous sedation) and the long downtime are huge. Not everyone is qualified to go under general anesthesia. Of course, a woman has to want to have implants and be larger to approach droopiness with an implant. And one of the most common problems I see in women with implants is women who really needed a lift, but instead chose to put in a big implant, making them larger than they wanted to be and stretching and thinning their tissue out more.

Liposuction is a very popular procedure during which a thin tube called a cannula is inserted through a tiny incision and is utilized to break up and suction fat cells from the body. Since its introduction several decades ago, the procedure has been improved with numerous innovations. Tumescent liposuction has become standard, and reduces bruising, swelling, and bleeding, and facilitates improved results. It involves infusing the treatment area with a large volume of a solution of lidocaine and epinephrine. This causes fat cells to become swollen and firm (tumescent), which makes them easier to break apart and remove. The solution also anesthetizes the area and temporarily shrinks blood vessels, limiting bleeding during the surgery. The tumescent technique enhances your comfort both during and after the procedure and reduces bruising and swelling.

Dr. Fouda Neel performs liposuction using IV sedation or general anesthetic, depending on the number of treatment areas and amount of fat being removed. He injects a large volume of tumescent solution into the areas of unwanted superficial fat to control pain and bleeding. This solution also makes the fat firm and easier to remove. Next, he makes tiny incisions near the targeted areas and inserts a hollow, stainless steel tube called a cannula into the fat. He moves the cannula in and out to break up the fat cells and suction them out of the body.
That being said, the regulations per US state, if considering to undergo a BBL there, impose different costs on the surgical facility that needs to be charged to patients. However, in densely populated areas with many plastic surgeons, whose surgical facilities are in particularly close proximity, the operating room charges don't vary that much within these areas. Therefore, if the majority of local operating rooms charge $500/hour and a surgeon is charging half that much, it should raise concerns that something is not being done at the facility; something that might compromise the safety of the procedure.
